Team News: Six changes for Chelsea on final day to take on Cardiff City

Chelsea boss Jose Mourinho makes six changes to his side for their final day clash with Cardiff City.

Cardiff City boss Ole Gunner Solskjaer has made two changes to his side to take on Chelsea on the final day of the Premier League season.

Craig Bellamy and Mats Moller Daehli come into the starting lineup from the 3-0 defeat to Newcastle United and replace Wilfried Zaha and Kim Bo-Kyung.

Chelsea boss Jose Mourinho has made six changes from the side that drew 0-0 with Norwich City last week, which ended the Blues' title challenge.

John Obi Mikel, Cesar Azpilicueta, Oscar, Fernando Torres, Tomas Kalas and Eden Hazard all come into the lineup this afternoon to take on the Bluebirds.

Cardiff: Marshall, Fabio, Caulker, Turner, John, Gunnarsson, Mutch, Whittingham, Daehli, Bellamy, Campbell

Chelsea: Schwarzer, Azpilicueta, Ivanovic, Kalas, Cole, Mikel, Matic, Salah, Oscar, Hazard, Torres
