Baba Rahman will reportedly travel to London to undergo a medical ahead of his proposed move to Chelsea.
The Augsburg defender's agent confirmed that he was in talks with the Premier League champions, while manager Jose Mourinho recently admitted that his side needs another left-back following the departure of Filipe Luis.
According to Ghana Soccernet, the 21-year-old will travel to England in the next few days to undergo a medical before finalising a contract.
The Ghana international is under contract with the Bundesliga side until 2019.
