The hosts took the lead on the opening drive of the match as Forbath converted a field goal from 31 yards, but that lead was wiped out later in the quarter when Ryan Succop kicked one of his own from 36 yards.
Forbath struck another from 31 yards to restore the Redskins' advantage, but it was the Titans who would go into the break with the lead as Charlie Whitehurst found Kendall Wright, who somehow skipped between a couple of defenders before advancing into the endzone for a 14-yard score.
Colt McCoy replaced Kirk Cousins at quarterback for Washington in the second half, and the move immediately paid off as the third-choice connected with Pierre Garcon for a 70-yard score on his first pass of the season.
Forbath added his third field goal of the evening from 27 yards to extend the advantage to six, but the Titans hit back midway through the fourth quarter as Derek Hagan combined with Whitehurst for a 38-yard score that took them back into the lead.
However, it was Forbath who would have the final say as he converted his kick in the dying seconds to end the Redskins' four-match losing streak and leave both them and the Titans with a 2-5 record for the season.
