The Belgian has been accused of barking an expletive in the youngster's direction when ordering him to return the ball during his side's 1-1 draw with Eintracht Frankfurt.
Former Chelsea player De Bruyne said after the game that he will provide the ballboy with a signed shirt as an apology, according to Bild.
However, the German FA has asked Wolfsburg to provide a written statement outlining the incident as part of its formal investigation.
De Bruyne scored the equalising goal against Frankfurt to rescue a point for his side.
