The Steelers were without starting quarterback Ben Roethlisberger for a third game, with veteran Charlie Batch filling in.
An Anquan Boldin touchdown was all that separated the sides in the first two quarters, with Justin Tucker and Shaun Suisham each kicking two field goals.
The Ravens remained seven points ahead going into the fourth quarter.
Tight end Heath Miller caught a seven-yard pass from Batch to level the scores, before Suisham kicked a 42-yard field goal with the final play of the game to give the Steelers the win.
The Ravens are still well-placed to clinch the division, with the Steelers in the hunt for a wildcard berth.
